New Patient Registration

We have an open list and welcome requests for registration from patients living in or moving to the practice area. We are also happy to see temporary visitors to the area should an urgent matter arise whilst away from home.

Alternatively, you can register by completing a registration form and a health questionnaire available from reception. It is helpful, but not essential, to bring your current NHS medical card. We now ask that all new patients provide us with proof of identity and address at the point of registration, for more information please speak to one of the receptionists.  

To save time you can download and print a registration form. You will need to complete this by hand and bring it to the surgery along with a completed new patient questionnaire (see link below).

New Patient Questionnaire

We recommend that new patients have a simple health check.  If you are not on any regular medication, this can be with the practice nurse or our health care assistant.  If you are on medication you will need to have your new patient check with the doctor so that we can set up your repeat medication.

Accountable named GP

In line with new regulations, each patient has a named, accountable GP on their clinical record, and this is the GP they are registered with. If you are unsure of who this is please ask at reception.

Your named GP has overall responsibility for your care but you can request an appointment with any GP.

If you have a preference as to who your named GP is please let us know and we will make all reasonable efforts to accommodate your wishes.

Updating your details

If you change name, address or telephone number, please let our receptionists know by calling in or writing to us. If you move outside the practice area you will need to find a doctor in your new area. There are instructions on what to do when you move to a new area on the back of your NHS medical card.  Even if you stay within our practice area, your dispensing status may change.  We are only permitted to dispense to people who live more than one mile from a pharmacy, as the crow flies.  Non-dispensing patients have to take their prescriptions to a pharmacy.
